Good news for cervix screening for patients within the Interim Federal Health Program (IFHP)
On an interim basis, HPV testing for individuals with IFHP coverage can be ordered at no cost through LifeLabs and Dynacare using the same processes used previously for ordering self pay cytology testing. Please use the attached (old) requisitions for now for Life Labs and Dynacare
Note that these requisitions are not yet updated. In the interim, so we do not have to delay offering cervical screening to these patients, when using these requisitions please note that patients will not have to pay and their signatures are not required. For further clarity, while we wait for the new requisitions, we can disregard the following lines on LifeLabs requisitions:
I acknowledge that I have informed my patient that this is a private pay test.
The patient will receive an invoice for the cost of the testing with instructions to complete payment.
By signing, I acknowledge that a payment to LifeLabs is required for the HPV/Cyto test. Patient Signature:
